Male fertility declines with age, more gradually than female fertility but measurably. The practical effects are that conception takes longer, miscarriage risk rises, and certain conditions in children become more likely with advanced paternal age, though those risks remain small in absolute terms. How does age affect sperm?

Sperm production continues throughout life, unlike the female egg supply, which is fixed at birth. That is why male fertility does not have an equivalent of menopause. What happens instead is a gradual decline in the efficiency of production.

Research on male aging and semen parameters shows semen volume, sperm count and motility declining gradually with age, with sperm morphology affected as well. The change is slow and varies considerably between individuals, which is why age alone tells you less about a man's fertility than a semen analysis does.

Sperm DNA damage

The more consequential change is to the DNA inside the sperm. Rates of DNA fragmentation rise with age, driven partly by oxidative stress and reduced efficiency in the production process, with the increase becoming more marked after 40. Fragmented DNA is associated with longer time to conception and higher miscarriage risk, and it is not something IVF or ICSI fully solves: selecting and injecting a single sperm bypasses problems of count and motility, but it does not repair the genetic material inside.

Does male age affect the chance of pregnancy?

Yes. Research on paternal age and time to pregnancy reports that men over 40 take longer to achieve a pregnancy than younger men. The effect is smaller than the female age effect and it compounds with it, so couples where both partners are older face the two together rather than separately.

This applies to assisted reproduction as well as natural conception. It is one of the reasons a fertility assessment should look at both partners rather than treating age as the woman's variable.

Are there risks to children of older fathers?

Research on advanced paternal age has associated it with a raised risk of certain conditions in children, including autism spectrum disorders, schizophrenia and some genetic conditions, with the increase becoming more apparent beyond 50.

These risks are small in absolute terms. A raised relative risk applied to an uncommon condition still produces an uncommon outcome. The point is not to alarm but to allow the question to be asked and answered properly rather than left out of the conversation. Discuss it with your specialist if it is a concern.

Can male fertility be preserved?

Sperm freezing stores sperm at its current quality for later use. It is standard before treatment likely to affect fertility, such as chemotherapy or radiotherapy, and it is also an option for men choosing to delay fatherhood. When should you seek advice?

The NICE guideline on fertility problems sets the usual threshold at twelve months of trying. Where the male partner is over 40, or the female partner is 36 or over, it is reasonable to seek advice earlier rather than waiting out the full year. A semen analysis is straightforward and is the only way to know where you stand.
