A randomized controlled trial in the American Journal of Obstetrics and Gynecology reported no cases of OHSS in the minimal stimulation group against 5.7 percent with conventional stimulation. What is conventional IVF?
Injectable gonadotropins are used over roughly two weeks to bring multiple follicles to maturity, so that several eggs can be collected in one retrieval. More eggs generally means more embryos, and more embryos means more chances from a single collection, including embryos to freeze for later.
The costs of that are a higher medication bill, more monitoring, and a higher risk of OHSS.
What is Mini-IVF?
Minimal stimulation IVF uses lower doses of injectable medication, oral medication such as clomiphene citrate, or a combination. Everything after the stimulation, the egg collection, fertilization, culture and transfer, is the same as conventional IVF.
Fewer eggs are collected. Medication costs less and monitoring is lighter, so the cycle costs less. The reduction in OHSS risk is the clearest documented benefit.

Who might Mini-IVF suit?

- Anyone for whom time is the constraint, since more eggs per cycle means fewer cycles.
- Patients wanting embryos to freeze for future transfers or siblings.
- Cases where genetic testing of embryos is planned, since testing needs enough embryos to be worth doing.
- Patients with diminished ovarian reserve who still respond to stimulation, where maximizing each collection matters.
One caution on the last point. Where testing will be done on the embryos, collecting fewer eggs and then filtering them further can leave nothing to transfer. That risk is worth discussing before committing to a combination of minimal stimulation and embryo testing.
How should you decide?
Ask your specialist what response they expect from your own results, how many cycles they anticipate under each approach, and what the total cost looks like across that number of cycles rather than for one. Cost per cycle is the figure that gets quoted; cost to a live birth is the one that matters.
